Plan HMO 20E             Plan 20 Enhanced. Pay a $20 co-payment for office visits. There is no co-                                                     payment for hospitalization.
Plan HMO 20Plan 20. Pay a $20 co-payment for office visits. There is $250 co-payment for         hospitalization.
Plan 25/40 Low Option Plan 25/40 Low Option. Pay a $25 co-payment for office visits and $40 co payment for specialist visits. There is $500 co-payment for hospitalization.

Healthy New York is a program offered by the State of New York designed to assist individuals and small business owners in providing their employees and their employees' families with the health insurance they need and deserve. In addition, uninsured workers whose employers do not provide health insurance may also purchase comprehensive coverage directly through the Healthy NY program. Although Healthy NY is available to small employers, sole proprietors, and uninsured working individuals, each group has its own distinctive set of eligibility criteria and participation rules.
